can a dentist be wrong about a root canal being needed

By Dr Anand Kulkarni (MDS, Endodontics) · Last reviewed 23 October 2026 · 526 words

The short answer

Yes. A dentist can occasionally recommend a root canal for a tooth that might have settled with a filling, or miss that the pain comes from a different tooth, a crack or the gums. Asking what tests support the decision, and getting a second opinion before an expensive plan, is reasonable and should never offend a good dentist.

A root canal is recommended when the pulp (the nerve and blood supply inside the tooth) is either inflamed beyond recovery or already dead and infected. Most of the time the evidence is clear. But the decision involves judgement, and judgement can be wrong in either direction: treating a tooth that would have healed, or delaying a tooth that needed it.

Where mistakes typically happen

  • Reversible versus irreversible pain. A tooth that hurts sharply with cold but settles within a few seconds often has a nerve that can recover once the decay is removed and the tooth is sealed. Pain that lingers for a minute or more, throbs on its own or wakes you at night suggests the nerve will not recover. In between is a grey zone.
  • The wrong tooth. Pain refers. A lower molar can make the upper teeth ache, and patients often point to the wrong one. Testing neighbouring teeth matters.
  • Pain that is not from the pulp. Gum abscesses, cracked cusps, high fillings, grinding and sinus pressure can all imitate pulp pain. A root canal fixes none of them.
  • A shadow on the X-ray that is not infection. Some normal anatomy and old healed areas can look suspicious on a single image.

Questions that show whether the decision is well-founded

  1. What tests did you do, and how did the tooth respond? (Cold test, tapping, biting, gum probing.)
  2. What does the X-ray show at the root tip?
  3. Is there any chance a filling alone would work? What would happen if we tried that first?
  4. Could the pain be coming from another tooth or from the gums?
  5. If we do the root canal, will the tooth then need a core build-up and a crown?

A dentist who answers these clearly, and shows you the X-ray, is giving you a sound basis to decide.

When waiting is reasonable, and when it isn't

If the signs are borderline, some dentists place a protective filling and review the tooth after a few weeks. That is a fair approach. But if there is swelling, a pimple on the gum, a dark area at the root tip or pain that keeps you awake, the nerve is usually beyond saving, and delaying risks a spreading infection. Facial swelling spreading towards the eye or neck, or difficulty swallowing or breathing, needs a hospital emergency department if we are closed; see our emergency page.

How we handle doubtful cases

At our clinics, Dr Avinash or Dr Rashmi sees you first. Where the diagnosis is uncertain or the tooth is complex, they can bring in a specialist from our MDS Endodontics department by prior appointment at Kharadi; an MDS specialist consultation is ₹1,500. The plan is written down so you can decide in your own time, not in the chair.

For reference, our current published fee for a root canal is ₹9,000, listed in our online store, with the final cost confirmed after examination. Many teeth can be done in a single sitting and some need two or more visits. Being asked to pay that for a tooth you are not sure about is exactly when a second opinion makes sense.

This answers a general question. It is not a diagnosis, and it cannot account for what is actually happening in your mouth. If something hurts, get it looked at — by us or by anyone.
