Symptom Breakdown

What you feel Most of the time, you feel absolutely nothing. A dental granuloma is a silent, chronic condition typically discovered as a dark shadow (radiolucency) at the tip of a tooth root during a routine dental X-ray. The offending tooth is completely dead and will not respond to hot or cold tests.
Why it happens A granuloma is a mass of chronically inflamed tissue. When a tooth nerve dies, mild bacterial toxins constantly leak out the bottom of the root. Instead of forming a violent, painful abscess right away, your immune system walls off the toxic leak with a tight ball of inflammatory scar tissue, dissolving the surrounding bone in the process to make room.
Clinical Advice
  • Do not ignore a dark shadow on an X-ray just because it doesn't hurt. A silent granuloma can slowly transition into a larger, bone-destroying radicular cyst, or suddenly flare up into a massive, acutely painful abscess if your immune system dips.
  • The infection is trapped *inside* the hollow tooth; antibiotics alone will never cure it because there is no blood supply inside a dead tooth to deliver the medicine.
Treatment Note Endodontic Therapy (Root Canal) to clean and seal the internal bacteria feeding the granuloma, or surgical Apicoectomy (removing the root tip) for persistent, unhealing lesions.
