Symptom Breakdown

What you feel A sharp, stabbing pain in your inner cheek or lip caused by a protruding metal wire. Alternatively, you might feel a loose bracket sliding freely along the archwire, or experience deep, generalized soreness in all your teeth for 2–3 days following an orthodontic adjustment (tightening).
Why it happens As your teeth shift into their correct alignment, the excess archwire is pushed backward out of the molar tubes, leading to poking. Loose brackets are almost exclusively caused by chewing hard, crunchy, or sticky foods (like ice, nuts, or caramel) that physically break the resin bond holding the bracket to the tooth enamel.
Clinical Advice
  • Always keep orthodontic wax on hand. Pinch off a pea-sized piece of wax, roll it into a ball, dry the poking wire with a tissue, and press the wax firmly over the sharp end.
  • Do not attempt to cut a thick archwire with household nail clippers or wire cutters. You risk swallowing the sharp fragment or shattering the bracket off your tooth.
  • For adjustment soreness, stick to a soft diet and use warm salt water rinses to soothe the inflamed periodontal ligaments.
